Ashwagandha (Withania somnifera) is an adaptogenic herb used as a supplement to support the body’s stress response and a calmer baseline mood. Lexapro (escitalopram) is a selective serotonin reuptake inhibitor, or SSRI, prescribed for depression and anxiety. The short answer on combining them: no dangerous interaction is documented, but because a prescription antidepressant is involved, this is a decision to make with your prescriber rather than on your own. How each one works
The two act through different systems, which is the main reason they do not obviously clash. Escitalopram blocks the reabsorption of serotonin at the synapse, leaving more available and gradually lifting mood over several weeks. Ashwagandha works elsewhere. Its withanolides are associated with a lower cortisol response and with GABA-like signalling, the pattern behind its reputation for taking the edge off stress. A randomised trial by Chandrasekhar and colleagues (2012) reported a meaningful drop in perceived stress and serum cortisol over 60 days at 300 mg of a root extract twice daily.
Because ashwagandha is not a meaningful serotonin booster, it does not raise the serotonin-syndrome worry tied to a precursor like 5-HTP. Serotonin syndrome is a serious reaction driven by excess serotonergic activity, described by Boyer and Shannon (2005), and the compounds that trigger it are the ones that flood the serotonin system directly. Ashwagandha is not one of them.
What the evidence on combining them shows
No trial has been designed around ashwagandha plus escitalopram, so direct evidence is thin. What exists is adjacent. A separate randomised study by Lopresti and colleagues (2019) found lower stress and morning cortisol in adults taking 240 mg daily for 60 days, and a trial by Salve and colleagues (2019) reported similar anxiety and cortisol changes without a safety signal. None of this tests the Lexapro pairing, but it explains why interaction checkers generally list no major flag.
The cautions are real even so. The likeliest issue is additive sedation. Beyond that, ashwagandha can raise thyroid hormone levels, which matters if you take thyroid medication, and rare reports link concentrated extracts to liver injury, summarised by the NIH LiverTox review. These are reasons to involve a clinician, not to panic.
What to look for in an ashwagandha supplement if you take an SSRI
If your doctor is comfortable with it, the label matters more than usual when you are already on a prescription. Choose a single-ingredient root extract with the dose stated per capsule and a named standardisation, such as KSM-66 (root only) or Sensoril, at a dose used in trials, roughly 240 to 600 mg daily. Third-party testing (NSF, USP, or Informed Choice) confirms the label is accurate, which matters when a clinician is signing off. Avoid combination formulas that bundle in 5-HTP, St John’s wort, kava, or other sedating or serotonergic extracts, since each one adds a fresh interaction to check. Cautions and interactions
This is a your-money-or-your-life topic, so the framing is deliberate: ashwagandha is not a treatment for depression or anxiety, and it is not a reason to change or stop your Lexapro. If you want to add it while on an antidepressant, the concerns are additive drowsiness, a possible shift in thyroid hormone levels, and rare liver effects with concentrated extracts. Watch for unusual daytime sleepiness, and stop and seek care if you notice yellowing skin, dark urine, or persistent nausea. Never adjust your escitalopram dose to make room for a supplement. Talk to your prescriber or pharmacist before combining the two, and bring the exact product so they can check it against everything else you take.
